- The Camper Health Information is now filled out and submitted online (by parent/guardian). We must have a new one done every year (which is according to the New Hampshire state law). To access this, you will need to go to the “register” section of our website (www.berea.org/register). If you have already created a login (when you registered online), use that same information. If you did not register online or create a login, you need to create a new account. Once you are logged in, you need to select the “2010 Summer Health Form (camper)” and then select an existing camper (from list on right hand side, if applicable) or add a new camper (if not on list). Now you are ready to enter the necessary information/answer the questions for that camper. Be sure to complete every field and hit submit at the end of the process. If you have more than one camper, you will need to create another “2010 Summer Health Form (camper)” for each additional child. Be sure to select/add the correct camper at the beginning.
